Health and Wellness Collaborative
A home is the anchor. Staying healthy is what makes it sustainable, so CIVC coordinates the care that keeps a veteran well.
What the collaborative covers
CIVC works alongside SIU School of Medicine, Lincoln Memorial Hospital, and the Logan County Department of Public Health to connect veterans with medical, dental, and mental health care rather than leaving them to navigate separate systems alone.
For veterans housed through the Tiny Homes program, the collaborative is the follow-through, the appointments, referrals, and check-ins that keep a new home from becoming an isolated one.
